PROCEDURES FOR ISSUANCE OF WITHDRAWL CARDS

In order to keep from being suspended, you need to request a withdrawl card when being laid-off, going on a
leave of absence, lengthy medical leave, or terminating your employment. You must request this withdrawl
within 90 days of your last day of work. It is your "responsibility" to obtain a withdrawl card. Failure to
request a withdrawl card may cause you to pay back dues and/or re-initiation fees. Remember that all
initiation fees and back dues must be paid before the withdrawl card can be issued (you must be paid
current and in good standing).
It is your option to get a withdrawl, if you do not wish to withdraw from the union it will be your responsibility to
pay your dues while you are off. You may continue to pay your dues up to six (6) months.
